Field And Player Insurance Forms
Every team needs to complete an application for field insurance every year.
This applies to: (1) existing team playing at the same fields, (2) existing teams moving to new fields, and of course also to (3) new teams. Despite the form talking about sending it to MSA, do not send your field insurance application to MSA because MSA will be confused when they get it from you versus getting it from the league. Save everyone the confusion, and email your field application form to GLWSLvp@gmail.com Field insurance contracts must be received by MSA 48 hours prior to any scheduled match or that match will be canceled. In order to have your insurance contract in at a timely manner, GLWSL requires you submit your paper work to the league ten (10) working days prior to the scheduled match to avoid complications.

If you get injured during a league game, err on the safe side and file a claim. Even if you have insurance through work. You might never need to spill over to this additional coverage, but it never hurts to open a claim and then later close your claim. Once you're done filling this claim form out, all player accident claim forms MUST be e-mailed to GLWSLvp@gmail.com to ensure they are processed. Any sent directly to MSA or to USASA by the player (or family member) will unfortunately get bounced for not having been first signed-off on by Great Lakes Women's Soccer League |
